Case Manager (Full Time)
Position Overview:
StageZero/CareOncology is looking for a Case Manager to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. The Case Manager is responsible for providing case management functions and exceptional service to customers, patients, families, and patient advocates. Case Managers work to facilitate patient care by assessing patient needs, evaluating treatment options, coordinating care. This position is to aid in enhancement of quality of life and promotion of well-being of patients, caregivers, and families. The Case Manager has the goal of engaging with our customers effectively to drive sales growth, improve customer retention and build brand loyalty.

StageZero Life Sciences is dedicated to the early detection of cancer and multiple disease states through our suite of blood-based diagnostic assays. As early pioneers in liquid biopsies, we developed one of the first blood-based biomarker tests for the early identification of Colorectal Cancer, ColonSentry®, and our next-generation pan-cancer test, Aristotle®, for simultaneously detecting signatures of 10 different cancers from a blood sample. These assays are based on our proprietary Sentinel Principle technology, the scientific concept that the circulating blood reflects, in a detectable way, what is occurring throughout the body. We continue to pursue the development of novel diagnostics assays using the Sentinel Principle to address unmet needs in early cancer detection and precision medicine.
StageZero Life Sciences operates a CLIA-certified and CAP accredited clinical reference lab based in Richmond, Virginia that offers our full suite of diagnostic tests. We have also recently initiated a partnership with Care Oncology Group which will offer a patient-driven, personalized care model to help reduce the risk of developing cancer and other chronic diseases. Through early interventions delivered using our proprietary advanced diagnostics and physician supervised telehealth oversight, patients will be able to take control over their health and their future.

Required Education, Skills and Experience:
- Bachelor’s Degree in Human Services or a related field with a minimum of two years of healthcare experience, preferably providing case management services
- Minimum of two years of sales or account management
- Professional or personal experience supporting individuals with terminal illness or challenging behaviors
- Exceptional Communication skills (written and oral)
- Ability to work under pressure and manage stressful environments
- Ability to meet monthly budgets and productivity goals
- Ability to quickly learn multiple cloud-based communication and EMR systems
- Proactive handling phone, video conferencing and email communications
- Self-starter, driven, problem-solver, and committed to achieving customer satisfaction
- Strong organizational skills with the ability to handle multiple priorities and demonstrate flexibility
- Proficient in Microsoft Office and Google Suite products
- Ability work independently with efficiency, accuracy, and professionalism with minimal supervision
- Ability to uphold HIPAA compliance
Preferred Education, Skills and Experience:
- Medical or Healthcare patient interaction
- Certification American Academy of Case Management
Essential Functions/Responsibilities (other duties may be assigned):
- Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation.
- Work with clients, family, friends, support networks and health care professionals to put care physician consultations in place
- Listen to clients’ concerns and provide counseling or intervention as required
- Evaluate clients’ progress periodically and adjusting as needed to improve outcomes
- Schedule appointments with the doctors using calendar software
- Communicate patient needs to a variety of care team members and follow up accordingly
- Provide information as regards treatment plans
- Prioritize assigned cases and assess the urgent needs of patients
- Respond to follow-up emails and phone calls
- Maintain thorough records and detail of all calls
- Build relationships with the patient and their support system assisting in identified areas of need.
- Follow up with discharged clients to ensure they were satisfied with services
